Pay: $23.19 - $29.46 an hour *NEW COMPETITIVE WAGES
Schedule: 12-hour Shifts; 36 hrs./wk. (variable days)
FAIRHOPE Hospice & Palliative Care, Home of The Pickering House is seeking a dayshift LPN to work with patients, families and other professionals to promote the philosophy of palliative care and the hospice concept. Our LPNs have a pivotal role in the hospice goals and daily care of our patients. FAIRHOPE looks to its LPNs to apply appropriate nursing care to foster the best possible experience for our hospice patients, their loved ones and care givers.

Responsibilities:
- Ensure quality and safe delivery of Hospice services to patients.
- Assist the patient with activities of daily living
- Implement current nursing practice following a patient assessment and an approved plan of care initiated by the registered nurse/case manager. Coordination of Hospice services, appropriately reporting the need for Hospice Interdisciplinary Team members, i.e. hospice aide, social worker, chaplain, etc.
- Assist patient/caregiver/family in working with other health care personnel in providing continuity of care.
- Provide appropriate pain and symptom management; evaluate patient’s response to treatments and/or medications.
- Document electronically any direct services provided and ongoing communication with patients, families and community providers.
- Remain HIPAA compliant at all times.
Qualifications:
- Graduate of an accredited school of professional nursing with a current LPN license from the Ohio Board of Nursing.
- Current BLS certification.
- Three years of recent nursing experience. End-of-life care and/or healthcare setting preferred.
- Ability to analyze and implement proper action is essential. Demonstrate great communication and social interaction skills, as well as a compassionate individual.
- Computer skills required.
- Physical Setting: Various healthcare settings. Some driving is required.
- Work Location: Service area coverage throughout Fairfield, Hocking, Perry and surrounding counties. Main office location is located in Lancaster, Ohio.

FAIRHOPE Hospice & Palliative Care, Home of The Pickering House is seeking Registered Nurse for the inpatient hospice unit, The Pickering House to work with patients, families and other professionals to promote the philosophy of palliative care and the hospice concept. Our RNs have a pivotal role in the hospice goals and daily care of our patients. FAIRHOPE looks to its RNs to apply appropriate preventative, therapeutic and rehabilitative nursing care to foster the best possible experience for our hospice patients, their loved ones and care givers.
We believe in a professional team approach and candidates must demonstrate great communication and social interaction skills, as well as, be a compassionate individual to work with other professionals and patients and families who face a vulnerable time in their lives.
The ability to analyze and implement proper action is essential. Computer skills required. Candidates must be a graduate of an accredited school of professional nursing and must have a current RN license from the Ohio Board of Nursing. Experience working with end of life care and/or healthcare setting preferred.
Responsibilities:
- Initiate and apply appropriate preventative, therapeutic and rehabilitative nursing procedures and techniques.
- Assess and evaluate patient’s status by:
- Reviewing and implementing the plan of care.
- Regularly re-evaluating patient needs.
- Revising the plan of care as necessary.
- Administer medications and treatments as prescribed by the physician. Responsible for counting of all controlled substances at each routine visit and log appropriate documentation of medication reconciliations.
- Teach the patient/caregiver/family self-care techniques as appropriate.
- Assist patient/caregiver/family in working with other health care personnel in providing continuity of care.
- Document electronically any direct services provided and ongoing communication with patients, families and community providers.
- Remain HIPAA compliant at all times.
Qualifications:
- Graduate of an accredited school of professional nursing, with a valid/current RN license from the Ohio Board of Nursing
- Possess current Basic Life Support (BLS) certification
- Three years of recent nursing experience; end-of-life care and/or healthcare setting knowledge is preferred
- An individual with the ability to identify areas for improvement and take necessary action
- Demonstration of excellent communication and interpersonal skills, as well as a caring attitude
- Working knowledge of computers
- Physical Setting: Inpatient care unit. Occasional work in additional healthcare settings. Driving may be required.
- Work Location: Inpatient care unit located on main campus in Lancaster, Ohio. Service area coverage throughout Fairfield, Hocking, Perry and surrounding counties.
